This volume is the product of the author's long experience with melanomas and sarcomas and to a lesser but significant degree with upper gastrointestinal cancers, colorectal and breast cancers. As such, it offers a “hands-on” practical guide to approaching complex soft-tissue tumors and for performing more extensive tumor resections based on over 40 years of surgical experience. It provides important details about the positioning of patients, incision types, and exposure which can be of paramount importance in the resection of certain tumors. The book contains important general surgical principles for approaching tumors in a variety of locations but also offers the detail necessary for the safe and oncologically sound resection of these malignancies. Moreover, this operative atlas contains specific information for procedures which are not as commonly encountered in surgical training, but can be invaluable in the management of patients with locally aggressive tumors, such as hemipelvectomy and its variants, sacral resections, and forequarter amputation. Through the use of multiple detailed illustrations, Atlas of Operative Procedures in Surgical Oncology serves as a valuable resource to the general surgeon or surgical oncologist in the operative management of patients with cancer in the abdomen, retroperitoneum, pelvis or extremities.

Vascular surgery has experienced remarkable growth and diversification in the last decade, especially with its embracing the disruptive endovascular technological revolution. Vascular surgeons remain the only group of specialists capable of total treatment of patients with vascular disease, providing medical, minimally invasive, and surgical therapy. Although vascular surgeons practice in all areas of the world, practice patterns in different areas naturally have both similarities and differences that reflect both regional patterns of disease, genetic characteristics of local peoples, cultural preferences for treatment, local political and economic situations, different access to resources and devices, as well as different training paradigms. Thus, the practice of vascular surgery has diversified and evolved in parallel in many areas, converging and diverging in many ways. Vascular Surgery: A Global Perspective provides the first review and comparison of the diversity of vascular surgery practice around the world. The book is grouped according to common vascular diseases such as aneurysmal and occlusive arterial disease, as well as venous and lymphatic disease and dialysis access. Each major disease topic includes multiple chapters written by expert specialists from around the world, each discussing their local approach to the disease and its treatment. Similarly, the status of vascular surgery practice is addressed, including discussion of the influence of payment systems on practice, patient access to the internet for information, training paradigms, and the legal system including malpractice. This textbook provides the first worldwide summary of the care of patients with vascular disease and will be of interest to a wide audience including vascular surgeons, vascular medical specialists, cardiologists, radiologists, internists, and family practice physicians.

As with the first edition, the second edition of this book covers the surgical anatomy of the abdominal wall, pathology of abdominal wall defects, such as hernias, enterocutaneous or entero-atmospheric fistulae, and indications for and surgical techniques used to reconstruct the abdominal wall from the practical stand point. In addition, through a number of illustrations, the placement of mesh in the abdominal wall reconstruction as well as manipulations of patient’s tissue including lateral compartment release techniques and other tissue transfer techniques are described in detail. The text also covers reconstruction of complex contaminated abdominal wall defects in patients with complex enteric fistulae, stomas, defects created after the excision of previously placed infected prosthetic mesh, and defects associated with acute tissue loss after severe trauma or necrosis of abdominal wall such as necrotizing soft tissue infections. Complex abdominal wall defects in the pediatric population and long-term outcomes and durability of these repairs are also addressed. The second edition of Surgery of Complex Abdominal Wall Defects is written by experts in their respective areas from around the world and has been updated thoroughly. As with the first edition, it will continue to serve as a guide for current practice for surgeons, including general, trauma, acute care, plastic and reconstructive surgeons.

This is a concise review of up-to-date concepts and techniques in the discipline of heart transplantation. It is a review and reference for practitioners managing patients with advanced heart disease, including patients with end-stage heart failure, mechanical circulatory support or transplant recipients. Heart failure is a major public health issue, with a prevalence of over 5.8 million in the USA, and over 23 million worldwide, and rising. The lifetime risk of developing heart failure is one in five. Heart failure carries substantial morbidity and mortality, with 5-year mortality that rival those of many cancers. As heart transplantation remains the best treatment option for patients with end stage heart failure, this primer will provide valuable information and management strategies for physicians caring for these patients. Also, due to continued shortage in donor organs, heart transplantation is a limited resource – which further underscores the importance of appropriately evaluating patients for transplant candidacy and managing their pre, peri- and post-transplant care for maximum benefit and best outcomes.

Gastrointestinal surgery is performed for a range of benign and malignant diseases in both elective and emergency settings. However, many of these operations are followed by complications, which need early detection so that they can be managed in a way that is rational, evidence-based and makes the best use of the available local resources. Complications after Gastrointestinal Surgery is an up-to-date and comprehensive reference book for practicing and aspiring gastrointestinal and general surgeons as well as for trainees and assistants. With contributions from more than 90 experts from 15 different countries, it offers a systematic description of the general aspects of surgical complications. It discusses institutional issues such as managing complications in limited-resource settings and the impact of centralisation and checklists. It also describes the ways to deal with specific post-surgery problems pertaining to different organs such as the oesophagus, the small and large intestines, pancreas and liver.

This book is a comprehensive and innovative guide to abdominal solid organ transplantation (SOT). It explains the main immunological problems involved in abdominal SOT, reviews indications for liver, kidney, and small bowel transplantation, and provides detailed descriptions of techniques for procurement and transplantation. In addition, technical, infectious, and immunological complications and their treatment are fully described. Special attention is devoted to the indications for and outcomes of combined kidney-liver and kidney-pancreas transplantation. Clear advice is provided on donor selection and donor safety, and the book concludes by examining medical disease and the risk of solid and hematologic malignancies after transplantation. Abdominal SOT has become the treatment of choice for various end-stage chronic diseases of the liver, kidney, and foregut. Improved knowledge of immunology, the introduction of new immunosuppressive agents, and advances in surgical techniques have all increased the number of potential candidates and led to better outcomes. It is therefore unsurprising that more and more surgeons, physicians, and scientists, as well as students and nurses, are becoming interested in abdominal SOT. This book will provide all practitioners with an excellent, up-to-date summary of knowledge in the field.

Provides a comprehensive, state-of-the art review of the current landscape of systemic and liver-directed therapy for malignant liver tumors Provides new insight into novel and evolving therapies in the areas of surgical technique and liver-directed arterial based and ablative therapies Written by experts in their fields

    About this book

The incidence of primary and metastatic liver tumors is rising and the therapeutic armamentarium for these tumors is expanding at a rapid pace. Surgery remains the primary treatment modality for these tumors. With improvements in our perioperative management and operative techniques a larger proportion of patients are able to undergo potentially curative liver surgery. In addition, indications and criteria for liver transplantation for both primary and metastatic liver tumors are constantly expanding. Unfortunately, many patients are not amendable to surgical resection or liver transplantation and therefore alternative liver-directed therapies are routinely being implemented as part of complex multidisciplinary treatment models.   However, these models are continually evolving as these regional therapeutic options continue to expand.

New embolization agents and techniques, as well as, improvements in radiation delivery, are all evolving in the background of improvement in systemic and targeted therapy options for these tumors.
This book provides a comprehensive, state-of-the art review of the current state of liver-directed therapy for malignant liver tumors. The book will serve as a valuable resource for not only surgeons and clinicians but for researchers with an interest in malignant liver tumors.  The book focuses on the therapeutic approach to primary and metastatic tumors providing an up-to-date review on surgical liver anatomy and new imaging techniques as well as patient selection and technical consideration for resection and transplantation. In addition, the book provides new insight into novel and evolving therapies in the areas of transarterial therapy such as: (i) bland embolization, (ii) chemoembolization, and (iii) radioembolization; as well as new radiation techniques including SBRT and proton therapy.

Furthermore, the expanding role of hepatic artery infusion therapy in primary malignant tumors is highlighted. Finally, integration of all these regional therapy options with upcoming and evolving systemic and targeted therapy regimens are integrated and discussed.   This book will serve as a very useful resource for clinicians and researchers managing and treating patients with these challenging malignancies.  It provides a concise yet comprehensive summary of the current status of the field that will help guide patient management and stimulate investigative efforts. All chapters are written by experts in their fields and include the most up to date scientific and clinical information.

Both the number of pediatric surgeons using minimally invasive surgery (MIS) and the number of operations involving MIS are increasing. This atlas of anatomy and operative techniques provides a thorough outline of the procedures commonly used in laparoscopic and thoracoscopic pediatric surgery. Step-by-step operative photographs are accompanied by explanatory text, and line diagrams are used to highlight the important anatomy as seen endoscopically. Alternative techniques, instruments, and steps are described when appropriate, and attention is drawn to highlights and pitfalls. While each chapter includes some basic background on the condition in question, the focus is very much on the operative procedures. This atlas will be of special benefit to those establishing a laparoscopic practice as well as to those in training who wish to embark on MIS. It will also serve as a supplementary atlas of anatomy and surgical technique for younger surgeons and medical students.

This book describes the diagnoses; staging and management of patients with colorectal liver metastases initially considered unresectable and portrays the different strategies to increase resectability along with their tactics and tricks. Colorectal carcinoma is the third most commonly diagnosed cancer in the world and according to recent cancer statistics around 1.23 million patients are diagnosed each year. Of these patients, approximately 50% will develop liver metastases during the course of their disease and around 15-25% are found to have stage IV disease at diagnosis. Liver resection has been recognized as the treatment of choice for these patients, offering overall 5-year survival rates of up to 50-60% and the only hope for cure. However, at diagnosis only 10-20% of these patients are possibly amenable to surgical resection with curative intent. The possibility to achieve an R0 resection is many times limited by the amount and quality of the future liver remnant (FLR), being posthepatectomy liver failure (PHLF) the most feared and severe complication after major liver resections. With the years, diverse strategies have been developed with the intention to increase resectability by increasing the future liver remnant and/or reducing tumor size, e.g. ALPPS. Along with these techniques, associated surgeries are developed including multivisceral resections, which broadens even more the resectability for patients.

This text is intended as a practical guide for physicians and surgeons who treat diseases of the breast, both benign and malignant. Thus, general surgeons, internists, family practitioners, gynecologists, surgical oncologists, radiation oncologists, and medical oncologists will find this book useful. The management of breast diseases requires a multidisciplinary approach, and this textbook therefore includes the views of a wide range of specialists from throughout the world. Today the treatment of breast diseases is largely predicated on the results of randomized prospective trials. The results of many clinical trials are discussed in detail, and readers will find this book to be a useful guide to the evidence-based treatment of breast diseases.
The revised edition provides a comprehensive update of the previous version of the book. Specifically, it contains 2 new chapters: “Molecular Classification of Breast Cancer” and “Breast Cancer Molecular Testing for Prognosis and Prediction”. In addition, since publication of the previous edition, the results of numerous adjuvant systemic therapy and neo-adjuvant systemic therapy trials have been published, and this edition provides a comprehensive overview of these trials and discusses how they have changed the management of breast cancer. Similarly, there have been, in the last few years, important new developments in the local therapy of breast cancer. This edition discusses these new developments and particularly recent developments and controversies concerning the management of the axilla in patients with primary breast cancer. New guidelines with respect to radiotherapy are also reviewed and discussed. Finally, all chapters in the book are thoroughly revised and updated to provide readers with current understanding of the biology and treatment of breast cancer.

This book provides a comprehensive overview of acute and chronic critical limb ischemia (CLI). Loss of an extremity, or a portion thereof, is not necessarily a life-ending process, but it is a debilitating experience whether involvement is of the upper or lower extremity. It reviews the epidemiology, pathophysiology, etiology, physical examination, imaging modalities, diagnosis, and treatment of limb ischemia. It investigates the most frequent as well as the more unusual etiological processes that may lead to the most dreaded concern of patients and families: amputation. The therapeutics of CLI has been significantly advanced through the multidisciplinary approach to the patient and disease, a focus that is explored in detail throughout the book. Surgical and endovascular treatment guidelines as well as medical therapy, wound healing, and long-term care are discussed. Reviews

“The book addresses all aspects of the pancreas and biliary tree from embryological development to the variety of pathologies. This is perhaps its greatest strength -- the ability to progress from the development of the foregut structures on through function and the treatment of common diseases. … This is a very nice addition to the field of pancreas and biliary surgery. … it is an excellent book that will serve its intended audience well.” (Peter Nau, Doody's Book Reviews, December 7, 2018)

The first version of this atlas was released as video-assisted major pulmonary resections were just emerging as a valid alternative to conventional techniques.  In this second edition, many different techniques have been described, depending on the use or non-use of an accessory mini-thoracotomy and on the use or non-use of endoscopic instrumentation and video display. One of these techniques is the totally endoscopic approach, in which only endoscopic instruments and monitor control are used. This is the technique that will be described in this atlas.

The purpose of this atlas is to describe each endoscopic pulmonary lobectomy and segmentectomy step by step, relying on brief technical notes and high-quality still pictures which are orientated and labeled to make them as comprehensible as possible. Each chapter is introduced by an anatomical background which is illustrated by three-dimensional reconstructions. Technical «tricks» and specific dangers are mentioned by pictograms. The technical descriptions of this atlas are based on the author’s technique, which can be different from other video-assisted approaches.

Our intent is that surgeons embarking in video-assisted major pulmonary resections-whatever the approach they use-can find helpful hints and take their bearings in this totally new vision of pulmonary and mediastinal anatomy.Compared to the previous version, all chapters have been rewritten, taking into account the progresses of the technique and the technology and some new chapters have been added. Most steps of the procedures are now illustrated, not only with pictures, but also with a video clip.
